Independent Living for Seniors (ILS), a Program of All-Inclusive Care for the Elderly (PACE), gives the frail elderly an alternative to nursing home placement.
Care from Skilled Professionals that Revolves Around You
Each ILS participant is served by a team of dedicated elder-care providers. Consistent assignments help providers become familiar with participants over time. They can be on the lookout for even subtle changes to help us get ahead of the curve on medical, functional, and psycho-social problems that need our attention. That translates to peace of mind for both ILS participants and their families. Care team members include:
- Primary care physicians and nurses
- Physicial, occupational and recreational therapists
- Social workers
- Home health aides
- Dieticians
- Drivers
Access to Services Made Easy
The ILS team works with the elderly and their caregivers to arrange for and monitor all the medical, social and daily living supportive services they need. We select services from the ILS network of providers. An individualized care plan is developed for each ILS participant by the interdisciplinary care team when they enter the program. Services include:
- Medical care and prescription drugs
- Home helper services for daily chores and personal care
- Transportation
- Adult day care
- Rehabilitation services
- Acute hospital care
- Nursing home care
- Help acquiring durable medical equipment and supplies
- Nutritional counseling and meals
