Hill Haven offers you:

Patient & VisitorFriendly and knowledgeable staff that will assist you in the admission process. Whether you are seeking admission from the hospital or home, looking for short term services or long term placement our staff is available to assist.
 
Tours are scheduled at the customer's convenience.

If you would like a tour of Hill Haven or information regarding nursing home placement, please contact the admissions office at (585) 922-2215.

General criteria for a patient to be admitted to Hill Haven  includes the following:

  • Age 16 or older
  • Medically stable
  • Patient Review Instrument, (PRI) completed by nurse.
  • PASAAR Screen - (completed by a social worker).
  • Current Medical Summary
