Transitional Care
When you are ready to leave the hospital, but not ready to return home, we can provide the care you need in a more relaxed, home-like setting. Our comprehensive “rehabilitation program” is located on a “separate unit” designed and staffed for medically complex patients.
All rooms have their own bathroom, TV, VCR, air conditioning and phone service.
Our Interdisciplinary Team includes:

- Social Work Services/Discharge planning
- Registered Dietician
- Physical, Occupational and Speech Therapies
- Psychiatric Consultation
- Therapeutic Recreation
Because of our affiliation with Rochester General Hospital, our staff is trained on the newest techniques to manage patients with complex needs.
