What to bring to your cardiology appointment
- Medical record information (labs, test results, x-rays, etc.) from other facilities
- A list (or the actual bottles) of all medications currently being used, both prescribed and over-the-counter
- Insurance card and any required forms such as a referral from your primary care physician or a completed authorization from your insurance company
- The names and address of all your physicians
What to bring for your cath lab procedure
- A responsible adult to drive you home (the patient should not drive for 24 hours after receiving sedation for a procedure)
- All medications currently being used, both prescribed and over-the-counter
- Insurance card and any required forms such as a referral from your primary care physician or a completed authorization from your insurance company
- An overnight bag, as there is always a potential for admission
- A copy of advanced directives (living willing, health care surrogate, medical power of attorney, etc); be sure to inform the staff if you have one
- Emergency contact information
- A list of past surgeries or procedures, dates and where they were done (open heart, cardiac cath)
DO NOT BRING valuables, money or jewelry – please leave those items at home or give to a family member for safe keeping.
