Joint Replacement Care Plan
A Successful Beginning: Pre-Operative Education
Our continuum of care begins by thoroughly preparing patients scheduled for joint replacement surgery. All patients are encouraged to attend a two-hour educational class hosted by our orthopaedic Clinical Navigator and featuring content developed by our orthoopaedic nurses, physical and occupational therapists and social workers.
Beyond leading these classes, our orthopaedic Clinical Navigator will also help guide you through the entire joint replacement process, allowing you to keep your focus where it's needed most: your healing and recovery. Learn more about total joint replacement education classes
Preparing For Success: Pre-operative Care
Before your surgery, you will be able to work with an RGHS social worker to pre-plan your discharge from the hospital to home and your post-operative rehabilitation. Whether you'll be recovering at home or in a short-term skilled nursing facility, you and your family will have the peace of mind of knowing everything is in place before your procedure.
When necessary, one or more of Rochester General Health System's many medical specialists will be able to consult on your plan to address any complex medical conditions that you may have.
Creating Joint Success: Operative and Hospital Care
As with everything we do, Rochester General Health System is committed to uncompromising excellence in the key component of a joint replacement patient's care: the joint replacement procedure itself.

The Next Stage of Success: Post-operative Care
After your procedure is completed, the high level of care continues. We utilize multi-disciplinary protocols for the prevention of post-operative complications such as deep vein thrombosis and infection, with your recovery monitored and assisted by a full team of caring professionals.
Your rehabilitation also begins right away with in-hospital therapy provided by our highly trained physical therapists - allowing you to speed your progress toward a successful recovery and leave the hospital more quickly.
Ensuring a Successful Ending: Recovery
Thanks to the plan laid out before your procedure, all the resource that you need to enjoy the shortest, most effective recovery possible will already be in place upon discharge.
We'll make sure that you have everything you need to restore function, mobility and strength to your affected joint.
