What Happens When I am Ready To Leave The Hospital
Where you go after the hospital will depend on what kind of therapy you need.
Some people are able to go home directly from the hospital and get therapy at home with a Home Care Agency. They can provide you with Physical, Occupational and Speech Therapy. The Rochester Rehabilitation Center offers outpatient Physical, Occupational and Speech Therapy to people who need all three therapies. They also have a support group for stroke survivors.
Rochester General’s Outpatient Center on Ridge Road offers Physical and Occupational Therapy. Speech Therapy is offered at Rochester General Hospital.
Some people may need to go to an inpatient rehabilitation program because it would be unsafe for them to go directly home.
The Acute Rehabilitation Unit at Rochester General Hospital offers intensive therapy, three hours a day. The program is coordinated by the Physiatrists (doctors who specialize in rehabilitation). The goal of the program is for the individual to return home.
Some people may not be able to participate in such an intensive program and would benefit from a rehabilitation program in a Skilled Nursing Facility. In this setting therapy is typically given for 1 ½ hours per day. The goal of these programs is to get the individual back home.
A minority of people may not be able to return home because of the effects of their stroke and will need long term care in a Nursing Home.

Stroke Follow Up Clinic
A Stroke Specialist will see patients at one, three and six months after discharge to check their progress and make sure they are continuing to work on decreasing their chances of having another Stroke. The Clinic will be held on the first Tuesday of every month.
