What to Bring to Your Appointment
Breast Images / Films
Please be sure to bring your breast images (films) with you to your appointment or request that the imaging group who performed your exam send the films directly to us. If you have any questions or concerns before your appointment, please feel free to call our office at (585) 922-4715.
Identification & Insurance
To ensure your safety and to ensure the accuracy and integrity of your medical records, you will be asked to provide proof of identity upon check-in. Please be sure to bring the following:
- Government issued ID card (i.e., driver's license, passport, etc.)
- Insurance card
