Clinton Family Health Center (CFHC) has been owned and operated by Rochester General Hospital since 1998 and serves a predominantly Hispanic population. In 2005, CFHC became a Federally Qualified Health Center.
There has been increased demand of CFHC due to recently implemented changes, and in 2006 the facility doubled its operating space in the Upper Falls Plaza; additional space utilized for expanded Family Medicine services, as well as introduction of OB/GYN services to this underserved area.
In the last 3 years, CFHC has garnered several awards for its work:
- In 2006, CFHC was 1 of only 9 programs nationwide that received the U.S. Dept. of Health and Human Services Innovation in Prevention Award. Also, data from the health center was instrumental in RGH receiving the 2005 Rochester Business Journal’s Innovations in Healthcare Award.
- In 2007, RGH received the prestigious 2007 HANYS Community Health Improvement Award for the work done at CFHC. The Hospital Association of New York State (HANYS) annually recognizes member organizations that have demonstrated improvement in the health and well-being of their community. CFHC was chosen from 59 programs across New York State that competed for the award.
The health center received the award for a series of changes they made starting in late 2003, as part of a “Reweaving the Safety Net” project that occurred between 2003 and 2006:- Instituted open access scheduling
- Implemented a team approach to managing patients
- Group medical care for patients with chronic illnesses
- Since 2004, the center has tracked the health outcomes of its patients and has showed dramatic improvement in several areas.
- The N. Clinton Business Association recognizes CFHC’s contributions to the wellness of that neighborhood and appreciates the anchor it has become.
- Being a walking neighborhood, many rely on their location and accessibility. The center is also multicultural, which makes them a huge asset.
