This check sheet is to help me organize the details for your interview visit. Please complete and submit it to me as soon as possible. 
 
I will be staying at the Holiday Inn, Airport. If you will be staying at the Holiday Inn Airport, you must make a reservation with your credit card telling them that you are with the Rochester General Hospital Internal Medicine Residency Program. When you check in your credit card will not be charged, **however, if you do not show up or cancel your reservation 72 hours in advance, you will be charged the hotel fee.** Please make your reservation early. The Holiday Inn, Airport will not gurantee a room immediately before your interview date.
 

I will be attending the dinner with the residents the night before my interview. Transporation will be provided from the Holiday Inn, Airport at 6:30 pm. If you are driving yourself, we will be sending you an email with the details closer to your interview date. Anyone traveling with you is welcome to attend.

Spouse or SO will be traveling with me and may be interested in going on the tour of Rochester at the end of the interview day. If so, arrangements will be made to have the person picked up at the hotel at 1:15 - 1:30 pm.

I will be able to go on the tour of Rochester. Please note: If you will not be able to go on the tour of Rochester. We will arrange a taxi to take you back to the hotel, airport, bus station or train station, however you will be responsible for the fare. Please be sure to bring your luggage with you to the hospital that morning if you will not be joining the tour.
