How ILS Works
ILS is a Program of All-inclusive Care for the Elderly (PACE) that gives the elderly and their families an alternative to nursing home placement. ILS provides medical care and support services for seniors in their homes, the community, and at the ILS (PACE) Day Center. ILS is the only PACE program covering the city of Rochester and surrounding towns.
The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible. PACE uses an interdisciplinary team approach. The team is comprised of doctors, nurses, social workers, therapists, nutritionists and home health care leader, who coordinate and manage the care of the participant in the program.
Delivering all needed medical and supportive services, the program is able to provide the entire continuum of care and services to seniors with chronic care needs while maintaining their independence in their homes for as long as possible.
