Case Management
The Case Management Program assists individuals in identifying needed services and helps them to access these services.
Services Provided
The Case Management Program provides the following services:
- Home visits
- Supportive individual, family, and group meetings
- Assistance with linkage to medication support services
- Individualized treatment planning with the clinical coordinator
- Coordination in crisis intervention
- Assistance with linkage to:
o Financial support systems
o Psychiatric services
o Social-recreational programming
o Vocational rehabilitation
o Transportation
o Housing

Monday - Friday, 8:30 am - 5:00 pm
